About the role

Join as an IRIS Consultant and empower individuals to live independently through the IRIS (Include, Respect, I Self-Direct) program.

Responsibilities

  • Building trusting relationships with participants
  • Developing IRIS Support and Services Plans
  • Maintaining regular contact and conducting visits
  • Assisting participants with IRIS eligibility and connecting with local agencies
  • Documenting all interactions promptly
  • Conducting annual Long-Term Care Functional Screens (as needed)
  • Working independently within a collaborative team

Qualifications

  • Bachelor’s degree in social work, psychology, human services, or related fields with at least one year of supervised experience with seniors or individuals with disabilities, or 4+ years of relevant social services experience, or current IRIS Consultant in good standing
  • Bilingual abilities are a plus
  • Strong commitment to participant autonomy and respectful service
  • Proficiency with smartphones, tablets, and computer systems
  • Knowledge of human behavior and community resources
  • Willingness to serve diverse populations with empathy

Requirements

  • Valid driver’s license and reliable transportation
  • Satisfactory driving record and auto insurance compliance

Schedule
