About the role
Join as an IRIS Consultant and empower individuals to live independently through the IRIS (Include, Respect, I Self-Direct) program.
Responsibilities
- Building trusting relationships with participants
- Developing IRIS Support and Services Plans
- Maintaining regular contact and conducting visits
- Assisting participants with IRIS eligibility and connecting with local agencies
- Documenting all interactions promptly
- Conducting annual Long-Term Care Functional Screens (as needed)
- Working independently within a collaborative team
Qualifications
- Bachelor’s degree in social work, psychology, human services, or related fields with at least one year of supervised experience with seniors or individuals with disabilities, or 4+ years of relevant social services experience, or current IRIS Consultant in good standing
- Bilingual abilities are a plus
- Strong commitment to participant autonomy and respectful service
- Proficiency with smartphones, tablets, and computer systems
- Knowledge of human behavior and community resources
- Willingness to serve diverse populations with empathy
Requirements
- Valid driver’s license and reliable transportation
- Satisfactory driving record and auto insurance compliance
