Social Services Consultant

About the role

We are seeking a compassionate and organized IRIS Consultant to support individuals with disabilities and older adults through Wisconsin's IRIS Program.

Responsibilities

• Build trusted relationships with participants
• Develop, update, and monitor individualized IRIS Support and Services Plans
• Conduct monthly contacts and quarterly in-person visits
• Promote self-determination, independence, and community inclusion
• Educate participants about program eligibility and resources
• Collaborate with community partners, healthcare providers, and service agencies
• Maintain accurate documentation and compliance standards
• Assist with long-term care assessments as needed
• Work independently within an interdisciplinary team

Qualifications

• Bachelor’s degree in a related field with at least one year of relevant experience
• OR four or more years of direct service experience
• OR current IRIS Consultant in good standing with a Wisconsin DHS exemption
