Shared Living-Host Home Coordinator/Case Manager

About the role

Mosaic at Home Coordinator is a salaried, daytime position with on-call responsibilities for providers and individuals on your caseload, including additional hours outside of normal business hours. The position involves conducting monthly home visits (minimum of 6 hours from Hastings) using an agency vehicle or your own, with mileage reimbursement.

Responsibilities

• Build Epic Connections: Develop strong relationships with individuals, their families, host home providers, and direct care workers.
• Be a Dream Champion: Assist individuals in expressing their goals, dreams, and needs, ensuring their voices are respected.
• Relationship Builder: Monitor and ensure the quality of relationships between individuals and their home providers.
• Arcitect Personalized Plans: Collaborate with the service planning team to create Individual Support Plans that align with individual wishes and regulatory requirements.
• Keep it Real in the Field: Oversee service delivery and contract compliance by conducting regular home and service environment visits.
• Be the Safety Net: Provide on-call support to providers and direct care workers when needed.

Requirements

• A Bachelor’s degree in a human service related field is required, with a minimum of two years of related work experience.
• Previous experience working with people with intellectual disabilities is a plus.
• A valid driver's license in good standing is required.
