Shared Living/Host Home Coordinator/Case Manager

Responsibilities

• Build Epic Connections: Develop rock-solid relationships with the individuals we support, their families, host home providers, and direct care workers. Provide exceptional customer service.
• Be a Dream Champion: Help individuals express their goals, dreams, and needs, ensuring their voices are loudly respected.
• Relationship Builder: Monitor the relationships between individuals and their home providers, ensuring a perfect, positive, and supportive match.
• Architect Personalized Plans: Collaborate with the service planning team to create Individual Support Plans, weaving together personal wishes with policy and regulatory requirements.
• Keep it Real in the Field: Oversee service delivery and contract compliance through regular home and service environment visits.
• Be the Safety Net: Provide on-call support to providers and direct care workers, solving problems quickly and effectively.

Schedule

This is a salaried, daytime position with on-call responsibilities outside normal business hours. Monthly home visits are required, which can be at a minimum of 6 hours from Hastings. The position is based in Hastings, with the option to use an agency vehicle or personal vehicle (mileage reimbursed).

Requirements

• Bachelor’s degree required.
• Minimum of two years of related work experience.
• Valid driver's license in good standing.

Qualifications

• Previous experience working with people with intellectual disabilities is a plus.
• Top-notch communication and relationship-building skills.
• Excellent problem-solving abilities and computer proficiency.
• Experience in behavior management and understanding of industry regulations.
