Responsibilities

  • Build rock-solid relationships with the individuals we support, their families, host home providers, and direct care workers. Exceptional customer service is key.
  • Help individuals express their goals, dreams, and needs, ensuring their voices are not just heard, but loudly respected.
  • Monitor the relationships between individuals and their home providers, ensuring the pairing is a perfect, positive, and supportive match.
  • Collaborate with our service planning team to create Individual Support Plans, weaving together a person's unique wishes with policy and regulatory requirements.
  • Oversee service delivery and contract compliance by conducting regular home and service environment visits.
  • Provide on-call support to providers and direct care workers when they need a helping hand or a quick problem solved.

Requirements

  • Bachelor’s degree required, along with a minimum of two years of related work experience.
  • Previous experience working with people with intellectual disabilities is a plus.
  • Valid driver’s license in good standing (required for home visits).
  • Top-notch communication skills, a knack for relationship-building, excellent problem-solving abilities, and computer proficiency.
  • Experience in behavior management and a good grasp of industry regulations.

Schedule

This is a salaried, daytime position. It includes on-call responsibilities for the providers and individuals on your caseload, which can include additional hours outside of normal business hours. You’ll conduct monthly home visits (a minimum of 6 hours from Hastings) using an agency vehicle or your own (with mileage reimbursement). This position is based in Hastings.
