Shared Living/Host Home Coordinator

Location: Hastings, Nebraska (Onsite position). Exempt, salaried role with on-call duties outside business hours and travel for home visits (up to 6 hours from Hastings). Agency vehicle or mileage reimbursement provided.

Responsibilities

• Build rock-solid relationships with the individuals we support, their families, host home providers, and direct care workers.
• Help individuals express their goals, dreams, and needs, ensuring their voices are heard and respected.
• Monitor relationships between individuals and their home providers to ensure a positive and supportive match.
• Collaborate with the service planning team to create Individual Support Plans that align personal wishes with policy and regulatory requirements.
• Oversee service delivery and contract compliance through regular home and service environment visits.
• Provide on-call support to providers and direct care workers when assistance is needed.
