Shared Living/Host Home Coordinator
About the role
Empowering Lives: Mosaic at Home Coordinator based in Hastings, NE (onsite). This exempt, salaried position includes on-call duties to support your caseload (which can be outside of business hours) and travel for home visits (up to 6 hours from Hastings). An agency vehicle or mileage reimbursement is provided.
As a Mosaic at Home Coordinator, you’ll be the ultimate bridge builder, ensuring the people we serve have a powerful voice and personalized support every single day. You’ll develop rock-solid relationships with individuals we support, their families, host home providers, and direct care workers, champion their goals and dreams, monitor relationships between individuals and their home providers, and oversee service delivery and contract compliance through regular home and service environment visits.
Responsibilities
- Build and maintain strong relationships with individuals supported, their families, host home providers, and direct care workers.
- Help individuals express their goals, dreams, and needs, ensuring their voices are heard and respected.
- Monitor relationships between individuals and their home providers to ensure positive and supportive matches.
- Collaborate with the service planning team to create Individual Support Plans that align personal wishes with policy and regulatory requirements.
- Conduct regular home and service environment visits to oversee service delivery and contract compliance.
- Provide on-call support to providers and direct care workers when needed.
- Uphold and demonstrate Mosaic’s values of belonging, connection, faithfulness, and grit in daily work.
Requirements
- Bachelor’s degree in human services or a related field (required).
- Minimum of two years of related work experience.
- Valid driver’s license in good standing (required for home visits).
- Top-notch communication skills and a knack for relationship-building.
- Excellent problem-solving abilities and computer proficiency.
- Experience in behavior management and a good grasp of industry regulations.
Previous experience working with people with intellectual disabilities is a plus.
