Shared Living / Host Home Coordinator

About the role

As a Mosaic at Home Coordinator, you’ll be the ultimate bridge builder, ensuring the people we serve have a powerful voice and personalized support every single day. This onsite position is based in Hastings, Nebraska, with travel for home visits (up to 6 hours from Hastings) using an agency vehicle or mileage reimbursement. The role includes on-call duties to support your caseload, which may extend outside of business hours.

Responsibilities

• Build rock-solid relationships with individuals we support, their families, host home providers, and direct care workers, ensuring exceptional customer service.
• Help individuals express their goals, dreams, and needs, ensuring their voices are heard and respected.
• Monitor relationships between individuals and their home providers to ensure a positive, supportive match.
• Collaborate with the service planning team to create Individual Support Plans, balancing personal wishes with policy and regulatory requirements.
• Oversee service delivery and contract compliance through regular home and service environment visits.
• Provide on-call support to providers and direct care workers for problem-solving and assistance.
