About the role
Seeking a compassionate Service Facilitator to support individuals and families through the Comprehensive Community Services (CCS) program.
Responsibilities
- Conducting functional and needs assessments
- Developing and implementing person-centered service plans
- Coordinating services and connecting clients with community resources
- Facilitating team meetings and collaborating with families and providers
- Maintaining accurate documentation and client records
- Advocating for clients using trauma-informed, strengths-based approaches
Qualifications
- Bachelor’s degree in Social Work, Psychology, Human Services, Counseling, Sociology, Child Development, or related field
- Experience with vulnerable populations preferred
- Strong communication, organizational, and relationship-building skills
- Ability to manage documentation, coordinate multiple services, and work independently
- Valid driver’s license, reliable transportation, and community travel ability
