Service Facilitator (Full-Time)

About the role

Seeking a compassionate Service Facilitator for the CCS Program to support individuals and families in accessing community resources and developing personalized service plans.

Responsibilities

• Conducting functional and needs assessments
• Developing and implementing person-centered plans
• Coordinating services and connecting clients to community resources
• Facilitating team meetings and collaborating with families, providers, and support networks
• Maintaining accurate documentation and client records
• Advocating for clients using trauma-informed, strengths-based approaches

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Sociology, Child Development, or related field
• Experience with vulnerable populations preferred
• Strong communication, organization, and relationship-building skills
• Ability to manage documentation, coordinate multiple services, work independently
• Valid driver's license, reliable transportation, and ability to travel within the community
