About the role

The Service Facilitator role involves partnering with individuals, families, and community providers to assess needs, develop personalized service plans, and coordinate support services that foster independence and well-being.

Responsibilities

• Conduct functional assessments
• Create person-centered plans
• Connect clients with community resources
• Facilitate team meetings
• Maintain accurate documentation
• Advocate for clients using trauma-informed, strengths-based practices

Qualifications

• Bachelor’s degree in Social Work, Psychology, Human Services, or related fields
• Preferred experience working with vulnerable populations
