Responsibilities
- Conduct functional and needs assessments
- Develop person-centered service plans
- Coordinate community resources
- Facilitate team meetings
- Maintain accurate documentation
- Advocate for clients using trauma-informed, strengths-based practices
Qualifications
- Bachelor’s degree in Social Work, Psychology, Human Services, Counseling, Sociology, Child Development, or a related field
- Experience with vulnerable populations
- Strong communication and organizational skills
- Ability to work independently
- Valid driver’s license with reliable transportation
