Service Facilitator (Full-Time)

About the role

The Service Facilitator role involves partnering with individuals, families, and community providers to assess needs, develop personalized service plans, and coordinate supportive services that foster independence and well-being.

Responsibilities

• Conduct functional and needs assessments
• Facilitate team meetings
• Connect clients to community resources
• Maintain accurate documentation
• Advocate for positive outcomes using trauma-informed, strengths-based approaches

Qualifications

• Bachelor’s degree in Social Work, Psychology, Human Services, Counseling, Sociology, Child Development, or a related field
• Preferred experience working with vulnerable populations
• Strong communication, organization, and relationship-building skills
• Ability to manage multiple services independently
• Valid driver’s license, reliable transportation, and community travel ability
