Service Facilitator (Full-Time)

About the role

Seeking a compassionate and organized Service Facilitator to support individuals and families through the Comprehensive Community Services (CCS) Program.

Responsibilities

  • Conducting functional and needs assessments.
  • Developing and implementing person-centered service plans.
  • Coordinating services and connecting clients with community resources.
  • Facilitating team meetings and collaborating with families, providers, and support networks.
  • Maintaining accurate documentation and client records.
  • Advocating for clients using trauma-informed, strengths-based approaches.

Qualifications

  • Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Sociology, Child Development, or related field.
  • Experience working with vulnerable populations preferred.
  • Strong communication, organization, and relationship-building skills.
  • Ability to manage documentation, coordinate multiple services, and work independently.
  • Valid driver's license, reliable transportation, and ability to travel within the community.
