Service Facilitator (Full-Time)

About the role

The Service Facilitator role involves partnering with individuals, families, and community providers to assess needs, develop personalized service plans, and coordinate services that foster independence and well-being.

Responsibilities

• Conduct functional assessments
• Create person-centered plans
• Connect clients to community resources
• Facilitate team meetings
• Maintain accurate documentation
• Provide trauma-informed, strengths-based advocacy to support positive outcomes

Requirements

• Bachelor’s degree in Social Work, Psychology, Human Services, or a related field
• Experience working with vulnerable populations preferred
