Service Facilitator (Full-Time)

About the role

Seeking a compassionate and organized Service Facilitator to support individuals and families by assessing needs, developing personalized service plans, and coordinating community resources.

Responsibilities

• Conduct functional assessments
• Create person-centered plans
• Facilitate team meetings
• Maintain accurate documentation
• Advocate for clients using trauma-informed, strengths-based approaches

Qualifications

• Bachelor’s degree in Social Work, Psychology, Human Services, Counseling, Sociology, Child Development, or related field
• Experience working with vulnerable populations is preferred
• Strong communication, organization, and relationship-building skills
• Ability to manage documentation, coordinate services, and work independently
• Valid driver’s license, reliable transportation, and ability to travel within the community
