RN Medical Transition Management - Onsite Virtual Care Center (Part-Time Nights)

Join a virtual healthcare team as an RN in a triage program. This role involves providing patient-centered care within a 24/7 virtual environment, supporting emergency department handoffs, scheduling, and care coordination for patients enrolled in an intensive program across various specialties like OBGYN or Pediatrics. Key Responsibilities Include Performing nursing duties according to federal, state, and organizational policies.Serving as a primary contact for patient care management and care transitions.Supporting education and care coordination across care continuum. Qualifications Graduation from an accredited nursing program (Associate's, Diploma, or BSN).Valid RN license in the state of practice or compact licensure.2-3 years of recent experience in Med Surg, ED, or Critical Care.Basic Life Support (BLS) certification; additional certifications such as ACLS, PALS, or ENPC may be required based on the role. Physical Requirements Include Ability to lift and move up to 50 lbs.Prolonged standing, walking, and physical mobility to perform duties effectively.
