RN Medical Transition Management - Onsite Virtual Care Center (Full-Time Nights)

Role: Registered Nurse (RN) in a virtual triage program, providing patient-centered care across OBGYN or Pediatrics specialties in a 24/7 environment. Responsibilities: Conduct telehealth assessments, serve as a point of contact for ED handoffs, facilitate scheduling, and support patients in an intensive care coordination program. Duties vary from education to care coordination, following all applicable regulations and policies. Qualifications & Skills Graduated from an accredited nursing program (Associate's, Diploma, or BSN).Current RN license in the state or compact licensure.2-3 years recent experience in Med Surg, ED, or Critical Care.Basic Life Support (BLS) certification; additional certifications like ACLS, PALS, NRP, or unit-specific credentials may be required.Physical ability to lift up to 50 lbs, stand, walk, and perform various physical tasks.
