RN Medical Transition Management - Onsite Virtual Care Center (Full-Time Nights)

Role Overview: Provide clinical evaluation and support for patients in the ED to determine eligibility for the ED to Home program, collaborating with bedside teams to facilitate outpatient care and assist with hospital follow-up scheduling. Support patients in avoiding unnecessary ED visits and hospitalizations, including transportation coordination. Qualifications & Skills: Registered nurse with a bachelor's degree (BSN), minimum of 5 years of diversified clinical experience, and active licensure in the practice state. BLS certification within 6 months. Preferred experience in OBGYN (Labor & Delivery, Women's Health) or Pediatrics (PICU, Urgent Care). Excellent communication, interpersonal, and analytical skills are essential, with the ability to work effectively in a virtual, fast-paced environment, prioritize tasks, and manage multiple patients. Key Responsibilities Evaluate patients to determine clinical appropriateness for ED to Home program.Coordinate outpatient care and follow-up appointments.Support patients in avoiding unnecessary ED visits through resource coordination.Work within a multidisciplinary team, demonstrating leadership and quick decision-making in stressful situations.
