RN Medical Transition Management - Onsite Virtual Care Center (Full-Time Nights)

About the role

This position involves evaluating patients in the Emergency Department to determine eligibility for the ED to Home program, collaborating with clinical teams to facilitate outpatient care, and supporting hospital follow-up appointments and transportation needs to reduce unnecessary ED visits and hospitalizations.

Responsibilities

• Assess patient needs

Qualifications

• Nursing degree (BSN)
• Valid state nursing license
• At least 5 years of diverse clinical experience
• BLS certification required within 6 months
• Preferred experience includes women's health, labor and delivery, pediatric urgent care, or emergency medicine
