RN Medical Transition Management - Onsite Virtual Care Center (P/T Nights)

About the role

Performs as an (RN) Nurse in the Mercy Virtual triage program in accordance with all applicable Federal, State, and Mercy rules, regulations, policies, and procedures, and performs all duties assigned by Medical Director, in the vAcute program, concerning patients and their care. These duties differ according to the assigned areas and range from education to care coordination. vAcute provides care on a 24-hour basis. Specialty may be OBGYN or Pediatrics.

Responsibilities

New initiative offering patient-centered care, a collaborative environment, and measurable impacts on the health of our communities. This position serves as a 24/7 point of contact for ED handoffs, facilitates scheduling, and supports patients enrolled in Mercy’s Intense Care Coordination program across the care continuum.

Requirements

• Education: Graduated from a school of nursing (Associate's Degree, Diploma, or BSN).
• Licensure: Must obtain and maintain a current RN license within the hiring state and/or compact licensure where nursing duties are performed, meeting all state board of nursing requirements.
• Experience: 2-3 years of recent experience in Med Surg, ED, or Critical Care.
• Certifications: Basic Life Support (BLS) certification through the American Heart Association or completion within 30 days of hire.
• Additional certifications that may be required based on position/unit: ACLS, NRP, ENPC, TNCC, CPHON, OCN, PALS, PEARS, S.T.A.B.L.E., Six Assessment & Care Modules, C-EFM (Fetal Monitor Certification), or other unit-specific certifications as required.
• Physical Requirements: Ability to push, pull, and/or lift 50 lbs. regularly; prolonged standing and walking during each shift; ability to grip, reach, bend, kneel, twist, and squat to perform duties.
