RN Medical Transition Management - Part Time Night

Role: Registered Nurse (RN) in a virtual triage program, providing patient-centered care across specialties such as OBGYN or Pediatrics. Responsibilities Serve as a 24/7 point of contact for emergency department handoffs and patient scheduling.Support patients enrolled in the Care Coordination program, ensuring seamless care across the continuum.Perform duties including patient education and care coordination, following all applicable regulations and policies. Qualifications & Skills Graduate of an accredited nursing program (Associate's, Diploma, or BSN).Current RN licensure in the state or compact licensure, maintained actively.2-3 years of recent experience in Med Surg, ED, or Critical Care.Basic Life Support (BLS) certification; additional certifications such as ACLS, PALS, ENPC, TNCC, or specialty-specific credentials may be required based on the role.Physical ability to lift up to 50 lbs, stand, walk, and perform various physical tasks. Key Duties: Provide timely triage, facilitate communication, support care transitions, and ensure adherence to safety and care standards in a virtual setting.
