RN Care Manager - Part Time

About the role

The Care Manager collaborates within an interdisciplinary team to assess, plan, advocate for, and coordinate patient care from admission to discharge, ensuring safe transitions post-hospitalization.

Responsibilities

• Oversee care management processes
• Optimize length of stay
• Maintain high standards of patient safety, experience, and quality of care through performance metrics and best practices

Requirements

• Graduate degree from an accredited School of Nursing
• Current licensure in the state of employment
• BLS (CPR) certification

Qualifications

• 2-3 years of experience in an acute care hospital setting
• Familiarity with Care Management or Utilization Management
• Certifications such as Case Management (preferred)
