Care Manager Role Overview The Care Manager supports effective transition planning and oversees patient length of stay, ensuring quality care, safety, and financial efficiency. They coordinate discharge processes from admission to discharge, collaborating with healthcare teams, and intervene with patients with complex psychosocial needs as assigned. The role promotes a seamless care model, enhances team communication, and optimizes performance using best practices. Qualifications Include Graduate of an accredited School of Nursing and current state licenseBachelor's Degree in Nursing2-3 years in acute care hospital settings preferredCare Management or Utilization Management experience preferredRequired certifications: BLS (CPR); Case Management certification is a plus Skills And Knowledge Excellent verbal and written communicationStrong assessment, time management, and critical thinking skillsProficient in computer applicationsAbility to interact effectively across all organizational levelsKnowledge of age-specific patient needs and care guidelines such as InterQual or Milliman
