RN Care Manager II - Weekend/PT

Role: The Care Manager facilitates patient discharge planning from admission to discharge, collaborating with the healthcare team to ensure seamless care, safety, and quality. Responsibilities: Coordinate patient throughput, intervene with high-risk or complex psychosocial needs, and promote best practices to optimize performance. Qualifications: Graduate of an accredited Nursing School, with a current state license and a minimum of a Bachelor's Degree in Nursing. Experience & Skills: Preferably 2-3 years in acute care settings, with care or utilization management experience. Must possess strong communication, assessment, critical thinking, and computer skills. Knowledge of care guidelines (InterQual or Milliman) is preferred. Certifications: BLS (CPR) required at hire or within 90 days; case management certification preferred. Additional: Ability to interact effectively at all organizational levels and adapt care to patient age-specific needs. Schedule: Day shift, working 32 hours per week.
