RN Care Manager II - Full-Time/Weekend

Summary Of The Care Manager Role The Care Manager facilitates the discharge planning process from admission to discharge, collaborating with the healthcare team to ensure seamless patient transitions. They may intervene with patients who have complex or high-risk psychosocial needs, ensuring care aligns with patient safety, quality standards, and best practices. Key responsibilities include overseeing length of stay, promoting team collaboration, and optimizing performance metrics within the Care Management model. Qualifications And Skills Graduate of an accredited School of Nursing with a current license in the state of employment.Bachelor’s Degree of Science in Nursing required.2-3 years of experience in an acute care hospital setting preferred.Care Management or Utilization Management experience is a plus.Mandatory BLS (CPR) certification at hire or within 90 days.Strong verbal and written communication, assessment, critical thinking, and time management skills.Advanced computer skills and ability to interact effectively at all organizational levels.Knowledge of age-specific patient needs and care guidelines, such as InterQual or Milliman Care Guidelines, is preferred. Additional Details This role involves working Saturday through Tuesday in 10-hour shifts, supporting a patient-centered, collaborative environment focused on quality care and continuous improvement.
