RN Care Manager II - Full-Time/Weekend

About the role

The Care Manager facilitates discharge planning from admission to discharge, ensuring seamless care, patient safety, and quality outcomes. They collaborate with healthcare teams, coordinate patient throughput, and promote best practices, especially for patients with complex psychosocial needs.

Responsibilities

• Perform assessments, develop care plans, and intervene as needed to optimize patient care
• Support transition processes, monitor length of stay, and ensure compliance with clinical guidelines such as InterQual or Milliman Care Guidelines

Qualifications & Skills

• Graduate of an accredited Nursing program with a current state license
• Bachelor's Degree in Nursing required
• 2-3 years of acute care hospital experience preferred
• Certification in BLS (CPR) required; Case Management certification preferred
• Strong communication, assessment, critical thinking, and computer skills
• Ability to interact effectively across all organizational levels and address diverse patient needs, including neonatal to geriatric populations

Schedule

This role involves working Saturday through Tuesday with 10-hour shifts.
