RN Care Manager II - Full-Time

Job Summary

Responsibilities

• Conducting comprehensive patient assessments and developing care plans
• Coinciding care to optimize patient safety, experience, and quality of care
• Managing length of stay and transition planning using performance metrics and best practices
• Advocating for patient needs and ensuring smooth post-hospitalization care

Qualifications

• Graduate of an accredited School of Nursing with a Bachelor's Degree in Nursing
• Current licensure in the state of employment
• 2-3 years of experience in acute care hospital settings preferred
• Certifications: BLS (CPR) required at hire or within 90 days; Case Management certification preferred
