RN Care Manager II - Full-Time/Weekend

Summary The Care Manager is responsible for facilitating seamless discharge planning from admission to discharge, collaborating with the healthcare team to ensure quality patient care, safety, and efficient throughput. They intervene with patients with complex psychosocial needs as assigned, promoting integration of best practices and performance metrics.
