RN Care Manager II - Full Time

Role Overview: The Care Manager facilitates discharge planning from admission to discharge, collaborating with the healthcare team to ensure seamless patient transitions and optimal care quality. Responsibilities: Coordinate patient throughput, promote team collaboration, intervene with patients having complex psychosocial needs, and support performance improvement using best practices. Ensure care aligns with patient safety, quality standards, and organizational values. Qualifications: Graduate of an accredited Nursing School, current licensure, and a Bachelor’s Degree in Nursing. Preferred 2-3 years of acute care experience, with knowledge of Care or Utilization Management. BLS certification required; Case Management certification preferred. Skills & Knowledge: Strong communication, assessment, time management, critical thinking, advanced computer skills, and ability to interact effectively at all organizational levels. Knowledge of age-specific patient needs and care guidelines such as InterQual or Milliman is preferred.
