RN Care Manager II - Full Time

The Care Management role focuses on facilitating discharge planning from admission to discharge, collaborating with the healthcare team to ensure quality patient care, safety, and efficient patient throughput. The Care Manager may also intervene with patients having complex psychosocial needs, promoting a seamless care model and optimizing performance through adherence to best practices. Position Details Qualifications include: Graduate of an accredited School of Nursing and current state licenseBachelor’s Degree in Nursing (minimum)2-3 years in acute care hospital setting (preferred)Care Management or Utilization Management experience (preferred)Basic Life Support (BLS) certification at hire or within 90 daysStrong communication, assessment, time management, critical thinking, and computer skillsAbility to effectively interact at all organization levels and address age-specific patient needsKnowledge of InterQual or Milliman Care Guidelines preferred Why Mercy?
