RN Care Manager - Full Time

About the role

The Care Manager conducts comprehensive assessments to determine appropriate healthcare services and benefits based on medical necessity. They provide education, facilitate transitions to clinical services, and advocate for enrollees to ensure timely, cost-effective, and quality care.

Responsibilities

• Coordinate services and complete assessments using approved tools
• Develop and monitor personalized care plans
• Document all activities in electronic records
• Observe enrollee conditions and follow up on changes
• Implement physician orders for treatments and medical equipment
• Adhere to regulatory standards and support optimal patient outcomes

Requirements

• Licensure as a Registered Nurse or BSN in New York
• BLS certification
• Valid driver’s license with personal vehicle
