RN Care Manager II - Full Time

About the Role

The Care Manager, as part of an interdisciplinary team, assesses, plans, advocates, and coordinates patient care from admission to discharge to ensure safe transitions and high-quality outcomes.

Responsibilities

• Facilitate effective transition planning
• Oversee length of stay
• Maintain patient safety and satisfaction using performance metrics and best practices

Requirements

• Graduate of an accredited School of Nursing
• Bachelor's Degree of Science in Nursing (minimum)
• Current state nursing license
• 2-3 years acute care hospital experience preferred
• Care Management or Utilization Management experience preferred
• BLS (CPR) certification required within 90 days of hire
• Certification in Case Management preferred
