RN Care Manager II - Weekend/PT

They intervene with patients who have complex or high-risk psychosocial needs as needed, promoting quality patient care, safety, and financial performance within a comprehensive Care Management model. Key responsibilities include overseeing patient throughput, encouraging team collaboration, and optimizing performance through adherence to best practices. Qualifications include a nursing degree from an accredited school, current license, and a bachelor's degree in nursing. Preferred experience encompasses 2-3 years in acute care, with knowledge of Care Management or Utilization Management. Required certifications include BLS (CPR), with Case Management certification preferred. Essential skills involve strong communication, assessment, critical thinking, and computer proficiency. Must be able to address the age-specific needs of diverse patient populations, including neonatal, pediatric, adult, and geriatric patients.
