Role Overview: Provides compassionate hospice and palliative care to terminally ill patients across all ages, utilizing the nursing process within regulatory guidelines. Key Responsibilities Conduct comprehensive assessments of patients and families, considering physical, emotional, spiritual, and environmental needs.Develop and update individualized care plans in collaboration with patients, families, and the interdisciplinary team.Administer medications, perform nursing interventions, and document findings accurately and timely.Monitor patient progress, recognize changes in condition, and communicate with physicians for optimal symptom management.Educate patients and families about care, end-of-life issues, and coping strategies.Oversee CNA performance, ensure quality documentation, and facilitate team communication.Participate in team meetings, care planning, and quality improvement initiatives.Support families through bereavement follow-up and community education.Assist in training new staff, students, and volunteers, and contribute to a positive team environment. Qualifications & Skills Associate degree in Nursing; BSN preferred.At least 1 year of RN experience; hospice or home care experience preferred or successful completion of a nurse residency.Valid RN license, driver's license, auto insurance, and acceptable driving record.Strong critical thinking, problem-solving, communication, and cultural competency skills. Additional Requirements: Demonstrates adherence to organizational policies, regulatory standards, and a patient-centered approach to care.
