Role Overview

Provides hospice and palliative care to terminally ill patients using the nursing process, offering support to families in the Hospice House. Ensures compliance with organizational policies and regulatory standards, practicing within the SC Nursing Practice Act.

Responsibilities

  • Assess patient physical, psychosocial, developmental, and environmental needs, considering emotional and spiritual aspects; monitor pain and symptoms; document findings.
  • Develop and update individualized care plans in collaboration with patients and families.
  • Educate patients and families, coordinate additional services, and create death plans aligned with patient wishes.
  • Perform nursing interventions, document in medical records, and communicate changes to physicians.
  • Oversee CNA performance, evaluate intervention effectiveness, and support family bereavement efforts.
  • Ensure continuity of care through effective communication with team members and external providers.
  • Document all clinical activities and manage discharge procedures post-death.
  • Perform admission, evaluation, consultation visits, and community education on end-of-life issues.
  • Assist in orientation and training of new staff and volunteers, and participate in projects and committees.

Qualifications

  • Associate degree in Nursing; BSN preferred.
  • Current RN license recognized in the practicing state.
  • Experience in acute care or hospice care preferred; no experience required.
