Registered Nurse (RN) - Hospice Inpatient, Full Time, Day

About the role

Provides hospice and palliative care to terminally ill patients of all ages using the nursing process, ensuring compliance with organizational policies and regulatory standards.

Responsibilities

• Assess patient physical, psychosocial, developmental, and environmental needs, considering emotional and spiritual aspects, and document findings.
• Develops and updates individualized care plans in collaboration with patients and families, offering education and support.
• Performs nursing interventions, monitors patient responses, and reports changes to physicians.
• Oversees CNA activities related to the care plan and evaluates intervention effectiveness.
• Supports families through bereavement follow-up and collaborates with team members for seamless care.
• Ensures accurate documentation, manages communication with vendors and agencies, and participates in team activities.
• Conducts admission and evaluation visits, educates the public on end-of-life issues, and trains new staff and volunteers.

Requirements

• Maintains current RN licensure, preferably with a BSN.
• Has experience in acute care.
• Hospice experience is preferred.
