Registered Nurse (RN) - Hospice Outpatient, Full Time, Day

About the Role

This position involves utilizing the nursing process to deliver hospice and palliative care to terminally ill patients across all ages, while providing supportive care to families. It requires adherence to organizational policies, licensure renewal, infection control, medication administration, and compliance with regulatory standards.

Responsibilities

• Conduct comprehensive patient assessments covering physiological, psychosocial, developmental, and environmental needs.
• Develop and update individualized care plans in collaboration with patients and families.
• Perform timely and accurate documentation.
• Communicate effectively with physicians and interdisciplinary team members.
• Implement nursing interventions, monitor patient responses, and evaluate progress toward goals.
• Provide education and bereavement support.
• Participate in team meetings and ensure quality care.
• Support team collaboration and participate in quality improvement initiatives.
• Assist with orientation and training of new staff.
• Uphold professional and ethical standards.

Requirements

• Associate degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred.
• Active RN licensure.
• At least one year of nursing experience (or completion of a recognized nurse residency program).
• Critical thinking, problem-solving, and strong communication skills.
• Knowledge of grief and loss processes.
• Valid driver’s license and auto insurance.
