Registered Nurse (RN) - Hospice Outpatient, Full Time, Day

About the Role

Provides hospice and palliative care utilizing the nursing process to terminally ill patients of all ages, supporting their families. Ensures compliance with organizational policies, licensure, infection control, and regulatory standards. Demonstrates clinical competency within the scope of practice and adheres to the SC Nursing Practice Act.

Responsibilities

• Assess patient physical, psychosocial, developmental, and environmental needs; develop and update individualized care plans in collaboration with patients and families.
• Perform thorough symptom and pain assessments, guiding interventions and medication management.
• Document findings accurately and communicate effectively with physicians and team members.
• Coordinate interdisciplinary care, oversee CNA interventions, and ensure quality and safe nursing practices.
• Provide patient and family education, including end-of-life planning and bereavement support.
• Participate in team meetings, quality reviews, and community education efforts.
• Evaluate patient responses to treatments and adjust care plans accordingly, considering cultural and developmental needs.
• Assist in orientation, training, and mentoring of staff, students, and volunteers.
• Maintain professional boundaries and foster respectful, collaborative team relationships.

Requirements

• Associate degree in Nursing; BSN preferred.
• Minimum of 1 year RN experience; hospice or home care experience preferred. Residency program completion can substitute experience.
• Valid RN license, driver’s license, and auto insurance.
