Registered Nurse (RN) - Hospice Outpatient, Full Time, Day

About the Role

Provides hospice and palliative care using the nursing process to terminally ill patients of all ages, supporting their families. Ensures compliance with organizational policies, licensing, and regulatory standards. Demonstrates clinical competency within the SC Nursing Practice Act.

Responsibilities

  • Conducts comprehensive assessments of patient needs, including physical, psychosocial, and spiritual aspects, and develops individualized care plans in collaboration with the interdisciplinary team.
  • Performs timely documentation, monitors patient progress, and adjusts care plans accordingly.
  • Educates patients and families, manages symptoms, and supports end-of-life planning.
  • Oversees CNA interventions, participates in team meetings, and ensures continuity of care.
  • Initiates bereavement follow-up and participates in quality improvement.
  • Provides patient education, advocates for patient rights, and communicates effectively with team members and physicians.

Requirements

  • Associate degree in Nursing and RN licensure.
  • At least one year of nursing experience or completion of a nurse residency.
  • Hospice or home care experience preferred.
  • BSN preferred.
  • Valid driver’s license with acceptable driving record.

Skills

  • Critical thinking and problem-solving.
  • Excellent communication and cultural sensitivity.
  • Knowledge of grief processes, compliance standards, and hospice care protocols.
