Registered Nurse (RN) - Home Health - Full-Time

About the role

We are seeking a compassionate Registered Nurse (RN) for a home health role focused on delivering patient-centered care through in-home visits, telehealth consultations, and administrative tasks. This role involves travel, documentation, patient education, and collaborating with healthcare teams to provide high-quality care in a community setting.

Responsibilities

• conducting assessments
• reviewing medications
• performing clinical interventions (e.g., wound care, medication administration)
• documenting patient encounters
• providing education
• supporting care transitions
• collaborating with interdisciplinary teams
• supporting urgent and episodic interventions while fostering positive relationships with patients, caregivers, and healthcare providers
• maintaining compliance with regulatory standards and ensuring accurate documentation
• participating in team meetings, ongoing training, and community engagement to enhance patient care

Qualifications and Skills

• Active state licensure (compact preferred) and CPR certification
• Minimum of 3 years in a direct patient care role, with experience in home care, hospice, palliative, or geriatrics preferred
• Proficiency in telehealth platforms, electronic health records, and Microsoft Office suite
• Strong communication, organizational, and decision-making skills
• Ability to work independently, travel locally up to 100%, and lift patients or equipment as needed
