Registered Nurse (RN) - Home Health - Full-Time

About the role

Providing high-quality, patient‑centered home health nursing care, including in‑home visits, telehealth consultations, and administrative tasks.

Responsibilities

• Conduct initial assessments, routine follow‑ups, and care coordination.
• Perform clinical interventions such as medication administration, wound care, and point‑of‑care testing.
• Document patient interactions accurately.
• Educate patients and caregivers.
• Support care transitions and quality initiatives.
• Collaborate with interdisciplinary teams.
• Participate in training and community engagement.
• Ensure compliance with regulations.

Qualifications

• Active state RN licensure (compact preferred).
• 3+ years of direct patient care experience.
• CPR certification.
• Prior telehealth or home care experience (preferred).
• Strong communication and organizational skills.
• Ability to work independently.

Skills

• Proficiency with electronic health records (EHR).
• Microsoft Office suite.
• Telehealth platforms.
• Cultural competence in diverse populations.

Additional Information

• No supervisory duties.
• Travel within the local area is required.
• Physical ability to lift and move patients.
