Registered Nurse (RN) - Home Health - Full-Time

About the role

A Registered Nurse (RN) specializing in home health care, responsible for delivering high-quality, patient-centered care through in-home visits, telehealth consultations, and administrative support.

Responsibilities

• Conduct comprehensive assessments
• Perform clinical interventions (medication administration, wound care, point-of-care testing, etc.)
• Document patient encounters accurately
• Provide education to patients and caregivers
• Support care transitions
• Collaborate with interdisciplinary teams to develop personalized care plans

Requirements

• No supervisory duties
• Requires local travel up to 100%
• Ability to lift up to 50lbs
• Work independently in various settings
• Emphasizes compassionate, ethical, and culturally competent care to support patients' health and well-being

Qualifications

• Active RN licensure (compact preferred)
• 3+ years of direct care experience
• CPR certification
• Proficiency with Electronic Health Records, telehealth platforms, and Microsoft Office

Skills

• Strong communication, organizational, and decision-making skills
• Experience with diverse populations and home care preferred
