Registered Nurse (RN) - Home Health - Full-Time

We are seeking a skilled and compassionate Registered Nurse (RN) for a home health role dedicated to delivering high-quality, patient-centered care. Key Responsibilities Include Conducting in-home patient visits, including assessments, health evaluations, and clinical interventions such as medication administration, wound care, and testing.Supporting care transitions, end-of-life discussions, and addressing quality gaps through screenings and education.Collaborating with interdisciplinary teams, including physicians and nurse practitioners, to develop and implement personalized care plans.Maintaining accurate documentation, ensuring compliance with regulations, and supporting telehealth consultations.Building positive relationships with patients, caregivers, and community providers. Qualifications And Skills Required Active state RN licensure; prior home care, hospice, palliative, or geriatrics experience preferred.Minimum of 3 years' direct patient care experience; CPR certification required.Proficiency with electronic health records, telehealth platforms, and Microsoft Office Suite.Strong communication, organizational, and decision-making skills.Ability to work independently, travel locally, and lift up to 50lbs. This role involves local travel, administrative duties, and participation in team meetings to ensure comprehensive patient care in a supportive, patient-first environment.
