Responsibilities

• Manage caseloads with routine visits, evaluations, and care starts.
• Develop personalized care plans based on assessments and physician orders.
• Educate patients on conditions such as CHF, COPD, Diabetes, and Wound Care.
• Collaborate closely with a multidisciplinary team including Physical, Speech, and Occupational Therapists.
• Utilize AI-powered tools for efficient charting to promote work-life balance.

Qualifications

• Missouri or Compact RN license.
• Valid driver’s license.
• Vehicle with insurance.
• Preferably experience in Home Health or Critical Care.
