Registered Nurse (RN) - Home Health - Full-Time

Join as a Registered Nurse (RN) Home Health Case Manager and provide hands-on patient care in clients' homes, coordinating services among healthcare providers, communicating with insurance companies, and supervising visiting nurses. Key responsibilities include managing caseloads with routine visits, evaluations, and care starts, developing personalized care plans based on assessments and physician orders, and educating patients on conditions such as CHF, COPD, Diabetes, and Wound Care. Collaborate closely with a multidisciplinary team including Physical, Speech, and Occupational Therapists. Utilize AI-powered tools for efficient charting to promote work-life balance. Qualifications include Missouri or Compact RN license, valid driver’s license, vehicle with insurance, and preferably experience in Home Health or Critical Care.
