Registered Nurse (RN) - Home Health - Full-Time

About the role

We are seeking a compassionate Registered Nurse (RN) for a home health role focused on delivering patient-centered care through in-home visits, telehealth consultations, and administrative tasks.

Responsibilities

• Performing comprehensive home safety evaluations and documenting findings.
• Conducting patient assessments, reviewing medications, and addressing health concerns.
• Providing clinical interventions such as medication administration, wound care, point-of-care testing, and other nursing procedures.
• Maintaining accurate and timely patient records and supporting care transitions, end-of-life discussions, and quality initiatives.
• Collaborating with interdisciplinary teams, including physicians, nurse practitioners, social workers, and caregivers, to develop personalized care plans.
• Supporting patient education and ensuring compliance with regulatory standards.
• Building relationships within the community and utilizing resources to meet patient needs.

Requirements

• Active state licensure; compact licensure preferred.
• Minimum of 3 years’ experience in direct patient care as an RN.
• CPR certification.
• Valid driver’s license with auto insurance.
• Ability to lift and move patients.
• Willingness to perform local travel and work independently.
• No supervisory responsibilities associated with this position.
