Registered Nurse (RN) - Home Health - Full-Time

We are seeking a compassionate Registered Nurse (RN) for a home health role focused on delivering patient-centered care through in-home visits, telehealth consultations, and administrative tasks. Key Responsibilities Include Performing comprehensive home safety evaluations and documenting findings.Conducting patient assessments, reviewing medications, and addressing health concerns.Providing clinical interventions such as medication administration, wound care, point-of-care testing, and other nursing procedures.Maintaining accurate and timely patient records and supporting care transitions, end-of-life discussions, and quality initiatives.Collaborating with interdisciplinary teams, including physicians, nurse practitioners, social workers, and caregivers, to develop personalized care plans.Supporting patient education and ensuring compliance with regulatory standards.Building relationships within the community and utilizing resources to meet patient needs. Qualifications Include Active state licensure; compact licensure preferred.Minimum of 3 years’ experience in direct patient care as an RN.CPR certification and valid driver’s license with auto insurance.Proficiency with electronic health records, telehealth platforms, and Microsoft Office suite.Excellent communication, organizational, and decision-making skills.Experience with diverse populations and a strong commitment to ethical practice. This role involves local travel, working independently, and physical ability to lift and move patients. No supervisory responsibilities are associated with this position.
