Registered Nurse (RN) - Home Health - Full-Time

Seeking a Registered Nurse (RN) for Home Health services in Topeka, KS. Role & Responsibilities Provide skilled nursing visits, including assessments, care planning, and patient monitoring.Develop, review, and revise individualized care plans per physician instructions.Administer treatments, observe patient responses, and report changes to healthcare team.Educate patients and families on care procedures and health management.Document care and patient conditions through electronic charting, supported by AI tools to enhance efficiency.Collaborate with multidisciplinary teams such as therapists and other healthcare professionals. Qualifications & Skills Current RN license in Kansas; LPN license acceptable with relevant experience.Preferred experience in Home Health or Critical Care.Valid driver’s license, vehicle, and current auto insurance.Ability to pass background checks, drug screening, and physical assessments.
