Registered Nurse (RN) - Home Health - Full-Time

About the role

We are seeking a compassionate Registered Nurse (RN) for a home health role focused on delivering patient-centered care in patients' homes.

Responsibilities

• Conducting in-home assessments, routine follow-ups, and care coordination under the direction of licensed providers.
• Performing clinical interventions such as medication administration, wound care, point-of-care testing, and other nursing procedures.
• Documenting patient interactions accurately and timely, supporting care transitions, and addressing health concerns.
• Providing patient and caregiver education, collaborating with interdisciplinary teams, and participating in care planning and team meetings.
• Supporting community engagement and maintaining professional standards, licensure, and certifications.
• Local travel to patient locations; physical ability to lift and assist patients; proficiency with electronic health records and telehealth platforms.

Qualifications

• Active state RN licensure, with preferred compact licensure.
• At least 3 years of direct patient care experience, including telehealth or home care preferred.
• CPR certification and a valid driver’s license with auto insurance.
• Strong communication, organizational, and decision-making skills, with experience working with diverse populations.

Schedule

Work hours are typically from 10am to 7pm EST, with no supervisory responsibilities.
