Registered Nurse (RN) - Home Health and Hospice Case Manager

We are seeking a dedicated Registered Nurse (RN) Home Health Case Manager, Hospice to provide compassionate hospice care in the community. Responsibilities include: delivering hands-on hospice care, supporting and educating patients and families, conducting assessments, collaborating with healthcare teams, and addressing physical, emotional, and comfort needs.Qualifications: active Kansas or Compact RN license; hospice experience preferred but training is available; valid driver’s license and auto insurance.Key skills: strong communication, empathy, ability to support families during difficult times, and teamwork.
