We are seeking a dedicated Registered Nurse (RN) Home Health Case Manager, Hospice to provide compassionate hospice care in the community. This role emphasizes presence, listening, and providing comfort, making a meaningful difference in patients' lives during life's most sensitive moments.
Responsibilities
- Deliver hands-on hospice care
- Support and educate patients and families
- Conduct assessments
- Collaborate with healthcare teams
- Address physical, emotional, and comfort needs
Qualifications
- Active Kansas or Compact RN license
- Hospice experience preferred but training is available
- Valid driver's license and auto insurance
