Registered Nurse (RN) Home Health Case Manager specializing in Hospice care.
Responsibilities
- Provide compassionate hospice care
- Support and educate patients and families
- Perform assessments and evaluations
- Collaborate with physicians and the hospice team
- Address physical, emotional, and comfort needs of patients
- Deliver hands-on hospice care
- Support families
- Ensure quality patient care through ongoing evaluations and team collaboration
Qualifications
- Active Kansas or Compact RN license
- Valid driver's license and auto insurance
- Hospice experience is preferred but training is provided
- Strong communication and compassionate presence are essential
