Registered Nurse (RN) - Home Health and Hospice Case Manager

Role: Registered Nurse (RN) Home Health Case Manager specializing in Hospice care. Responsibilities: Provide compassionate hospice care, support and educate patients and families, perform assessments and evaluations, collaborate with physicians and the hospice team, and address physical, emotional, and comfort needs of patients. Qualifications & Skills: Active Kansas or Compact RN license, valid driver’s license and auto insurance. Hospice experience is preferred but training is provided. Strong communication and compassionate presence are essential.
